New Mexico Medication Therapy Management Collaborative The New Mexico Medication Therapy Management (MTM) Collaborative is aimed at improving outcomes for people with diabetes, hypertension, hyperlipidemia and congestive heart failure. The primary objective of the collaborative is to assess the impact of face-to-face MTM on adherence of medications used to treat the specified conditions. The collaborative involves pharmacists from health plans and community pharmacies statewide working together for 12 months to implement differenct levels of MTM (face-to-face, phone, materials distribution), test system changes, implement information systems for collecting data, and share learning. The major components of the collaborative are prework activities, learning sessions, action periods and the outcomes congress. NMMRA has applied for continuing pharmacy education credit for the collaborative sessions. Learning sessions are the major interactive events of the collaborative. Through plenary sessions, small group discussions and team meetings, attendees can learn from national experts, collaborative faculty and colleagues; receive individual coaching; gather knowledge; share experiences and collaborate on improvements; and problem-solve barriers. Learning Session 1 Learning Session 1 was held November 3-4, 2007, at the University of New Mexico College of Pharmacy. Attendees came from throughout the state to be trained on the basic tenets of face-to-face MTM and the clinical conditions of focus. The New Mexico Pharmacists Association is accredited by ACPE as a provider of continuing pharmacy education. Completed evaluations must be submitted to the New Mexico Pharmacists Association to request statements of credit.

Learning Session 2 Learning Session 2 was held in combination with the semi-annual meeting of the full New Mexico Prescription Improvement Coalition on March 20, 2008. The following presentations are of particular relevance for MTM Collaborative participants: The Asheville Project: A Community-based, Long-term Medication Therapy Management Program for Chronic Disease, by John Miall, Miall Consulting, Special Consultant to the American Pharmacists Association Foundation Instructions: New Mexico MTM Collaborative Recordings, Learning Session 1 Action periods are the time between learning sessions. During action periods, teams work within their organizations to test and implement changes aimed to improve medication management. Teams share the results of their improvement efforts through progress reports, a listserv and periodic conference calls. Participants will submit their data via a secure Web portal to collaborative faculty. Participation in action periods includes learning session attendees as well as other team members and supporters in the settings. The outcomes congress is an opportunity for participants to share findings and achievements, highlight the accomplishments of the teams, and present effective models of care.
